In a mixing bowl, combine the brown sugar, honey, Dijon mustard, apple cider vinegar, ground cinnamon, ground cloves, black pepper, and salt. Mix well until you have a smooth mixture.
Place the fully cooked ham in the Crock Pot, fat side up.
Pour half of the glaze over the ham, using a basting brush to spread it evenly.
Cover the Crock Pot and cook on low for 6 hours, basting the ham with the remaining glaze about halfway through the cooking time.
Once cooked, carefully remove the ham from the Crock Pot and let it rest for about 15 minutes before carving.
Slice the ham and serve with the delicious glaze drizzled on top.
For a more caramelized finish, place the sliced ham under the broiler for a few minutes after cooking.
